Dive's question (quiz) feature can be used for inspection sheets and checklist operations by using "pass/fail evaluation" — not just for education-focused correct/incorrect answers. Even when abnormalities are found, the SOP does not stop; instead, answers are recorded as "abnormal". Later, you can check "who answered what and when" in the viewing records of the SOP under the [Work records] tab.
Available plans: The question feature itself is available in Core plan and above. To view answers to questions in work records, a Core plan or above is required. CSV export of work records is available in Pro plan and above.
How to create "Continue when unexpected" questions
When you select the question type, a "Continue when unexpected" checkbox (previously "Use for pass/fail evaluation"; the behavior is unchanged) appears directly below it. When enabled, that question judges "normal/abnormal" instead of "correct/incorrect".
- Even if abnormal, the SOP does not stop but continues
- Answers are recorded as "abnormal" and can be checked in work records later
- It is ideal for inspection sheet and checklist operations where you "honestly report any abnormalities found and continue"
The supported question types are the following two:
Multiple choice (Yes/No, Selection, Multiple selection)
- In the question card, set the type to [Yes/No], [Selection], or [Multiple selection]
- Enable the [Continue when unexpected] checkbox
- A "Normal" column appears in the options table. Check the normal options (at least one required)
- If you select anything other than normal options, it will be recorded as "abnormal"
Numeric input
- In the question card, set the type to [Numeric input]
- Enable the [Continue when unexpected] checkbox
- Enter the minimum to maximum values for the "Normal range"
- If a value outside the range is entered, it will be recorded as "abnormal"
* When pass/fail evaluation is OFF, it operates as before: "Correct range" (quiz behavior that only allows passage when correct).
Using with branching logic
Questions with pass/fail evaluation can be combined with branching logic. In the numeric input branching editor, if you set the condition "Range" to [Match question settings], it will automatically sync with the normal range on the question side (when you change the question side, the branching side follows). You can quickly set up a flow where "if normal, go to the next step; if abnormal, go to the abnormality handling procedure".
Check work records
For each SOP, you can check the record of who viewed when and what they answered under [Work records] in the viewing records of that SOP (see Check viewing records by SOP).
Places to open work records
- Step list: From the […] menu of the SOP, select [Viewing records], then switch to [Work records]
- SOP details: From [Viewing records] at the top of the screen, select [Work records]
- Usage Report: In the [Viewing records] tab, switch the breakdown to [SOP], press the SOP name, then switch to [Work records]
- Personal page [Viewing records] tab: Press the SOP name to see the work records for that person. Press a row in the view history to open details for that session
What you can check in work records
- Rows: One work session (who viewed when)
- Columns: Each question in that version

Cells: Answer value + OK/NG chip
- Green (OK): Passed with normal option/normal range value
- Red (NG): Passed with abnormal option/out-of-range value (= abnormality present)
- Check: Confirmation-only question with no pass/fail setting (no correct answer or range set)
- Numeric: Measured value when no pass/fail range is set for numeric input
- AI badge: If AI check is being used for a photo submission question, AI reference findings (AI ✓/AI !/AI ?) appear in the cell (Pro plan and above). The answer judgment is not overwritten.
- Version dropdown: Initially displays records from all versions (which version the record is from is shown in the [Version] column). Select a specific version to show only records from that version
- Status summary: Displays the status and count for each work session at the top. A legend of symbols also appears above the table
- Click a row to open a detail drawer for one work session
- Status filter: All statuses are listed with counts; statuses with 0 entries cannot be selected
- Total (this period): Below the table, the count of abnormalities by question is always displayed (for numeric input, the average)
CSV export (Pro plan and above)
Click the [Export] button in the top right of the table to export work records as a CSV file. You can store it as a deliverable of an inspection sheet or checklist, or use it as the source for importing into external document formats.
Who can view work records
- Owner / Team Administrator: Can view all records for the target SOP within the team
- Group Administrator: Limited to records of users under their managed group (if not part of any group, only their own records)
- Created user / View-only user: Can check only their own records from [Viewing records] in the SOP details
- Guest: Cannot view
Example uses
- Pre-shift inspection: Compose a group of Yes/No questions as "Continue when unexpected". Even if abnormalities are found, the operation does not stop and records remain as an inspection sheet
- Specification check for numeric values: Set the normal range for measured value input questions. Out of range = abnormality; within range = OK, all listed
- Different procedure for abnormalities: With pass/fail evaluation + branching logic, automatically jump to an "abnormality handling manual" when abnormalities are detected
- Store as audit records: Export CSV monthly, use as a document for internal audits and ISO operations

Work session statuses
Each work session is divided into the following statuses based on how many questions are answered. The meaning of symbols can be confirmed in the legend above the table and in the display when hovering over a cell.
- Completed (✓ green): Viewed to the end and answered all questions
- Completed (with stumbles) (△): Answered but some questions were wrong
- Completed (with abnormalities) (✕): Answered but some questions were recorded as "abnormal"
- Completed (partially answered) (✓ gray): Viewed to the end but some questions were not answered
- Completed (no answers) (⊘): Viewed to the end but did not answer any questions
- Incomplete (⊘): Did not view to the end
- Reference (no questions) (◌): A session where the SOP was read in reference mode, skipping questions. Not counted in inspection totals or training course completion
"Completed (partially answered)" and "Completed (no answers)" are states where a completion report has been submitted. They are distinguished as separate states from "Completed" where all questions were answered. Because inspection sheet operations where only some questions are answered and then ended are allowed, work sessions can proceed to completion reporting even if some questions remain unanswered (unanswered questions are displayed in a list; pressing them takes you to that step).
"Unanswered" display
When an answer is not recorded for a question cell, all are displayed as "Unanswered". There is no way to distinguish from the record whether the step was reached but not answered or not reached at all. Additionally, because some uses involve answering only questions from the table of contents, a distinction is not made based on whether the step was reached.
